It can't happen in a standard game.
 I have a house rule that in a multi-player game then once either chit has come out of the pot then it stays out so that it only happens once per game. There are 36 damage chits - one red side of which is a FTE - that's a 3% possibility. PK is two red, that's a 6% chance and a blue which is another 3% chance. I'm currently running at about 70% in 120 games of either of these as a way of being killed. That has got to be a programming error, unless I've been hit that many times
