I think that if he's just "Dead", perhaps he had a heart attack or just fell out of the back-seat. When he is "Killed" it implies that (probably) an enemy shot him to death. Of course the pilot may have dispatched him if he didn't turn down his damned "Boom-Box" back there! (My observer tends to play Over There over and over and over. It can just drive you NUTS!)
