To answer the first question, I do not have a problem with slow starting, though I think purposely avoiding all contact in order to let your opponent run out of fuel is unsporting. On the other hand, if you are flying an inferior plane against two or more better planes, say the SPAD XIII against the Fokker DVII, flying defensively is not only ok, but about the only way you can survive.
I have no problem shooting down others who run out of fuel or have no engine. I will make exceptions for pilots I often fly against and respect however. I do not get angry when others shot me down when I am in that situation.
