Well, virtually all air combat wargames include altitude rules, at least as an optional expansion on the basic games. From all the pilot memoirs and diaries I've read, altitude was certainly a key factor in actual combat, not just in preliminary maneuvering. Finally, given the option for much bigger map sizes, than in the original game, I feel the larger scope of such lends itself well to including altitude as an option.
Canvas Eagles includes altitude rules and plane data we could easily adapt. Ideally, I think altitude rules would be an additional optional item one could select when creating scenarios.
